"Facing the Abyss: Medical Ethics and Alzheimer's Disease" contains several useful questionnaires to assist older adults and their families in planning for future medical care. Some of the topics covered in the questionnaires include: personal relationships, overall attitude toward life, thoughts about illness, wishes concerning specific medical procedures, etc. Included is a section with intergenerational discussion questions.
