Toronto Hospital Memory Clinic

The Toronto Hospital Memory Clinic provides a multidisciplinary assessment of persons who suffer from dementia, as well as ongoing management and supportive therapy for caregivers. The team includes a geriatric psychiatrist, neurologists, neuropsychologist, geriatrician and social worker. Note: a doctor's referral is required.

The goals of the clinic are to:
  1. diagnose the dementia
  2. provide ongoing psychiatric and psycho-social care for the patient
  3. educate and support the patient's caregivers
  4. assist with obtaining appropriate community services and advocate on behalf of patients and caregivers
  5. serve as a base for research into the causes and treatment of dementia
